This Policy will summarise how the Domiciliary Care Service Provider liaises and co-ordinates with providers of other specialist clinical and medical services in order to develop a unique and effective Care Plan for the service user:

1. The staff at Prime Recruitment Ltd are dedicated to providing an excellent service of care and welfare to our service users. We achieve this through a thorough assessment of needs which will provide the basis of a Care Plan that gives the service user a first-class quality of life. Policy No 3002 refers.

2. The Care Plan is unique to the service user, and may involve input from other agencies that can provide specialised care services that the Organisation is unable to deliver. The following will provide a list of additional agencies that may be called upon on a confidential basis to contribute to the Care Plan according to need (the list is not exhaustive):

  • Advocacy Services

  • Audiologist

  • Chiropodist / Podiatrist

  • Clinical Psychologist

  • Community Learning Disabilities Professional

  • Community Mental Health Professional

  • Community Psychiatric Nurse (CPN)

  • Continence Management Service

  • Dentist / Orthodontist

  • Dietitian

  • District Community Nurse

  • Drug & Alcohol Treatment Services

  • Occupational Therapist

  • Optician

  • Pain Management Services

  • Psychiatrist

  • Physiotherapy Services

  • Safeguarding Teams

  • Social Worker

  • Specialist Falls Team

  • Specialist Medical / Clinical Services

  • Speech & Language Therapist

  • Tissue Viability & Wound Treatment Nursing Services

  • Translation Services (where the service user’s first language is not English)

  • Other

3. Form No 3-018 will provide a record of external agencies used, together with a summary of care services provided, and will be retained as part of the service user’s Care Plan records.
